Q: How do you get the hot therapy as this only shows how to use cold by freezing packs?

The ice/heat packs can be easily removed. For the heating therapy I put them in the microwave.

Q: Will this work for icing both the back and right side of my left knee simultaneously?

Yes you can use this it basically will wrap ice around the entire knee. I have this one because it runs a little bit small and I use it right below the knee around the calf. I use a different one to go around my knee because my knee is a little bit larger due to multiple surgeries and what I'm trying to recover from. so this one runs a little bit on the smaller side it's probably more designed for a woman but a small to average size man could use it very effectively as well. I've used it on my knee in the pinch but I prefer the one that's a little bit larger.

Q: Says it fits up to 21" knee circumference. Would this fit me if Just above my knee, I guess would be considered lower portion of my thigh is 21"?

All I can say is that above my knee is just under 20", fits fine. Good luck.
